Use the chart below to locate the number of people in your household, and then the corresponding income level that matches the annual earnings of your household. At the top of the income column you’ll find the cost of your visit.

I have insurance, can I still use the sliding fee scale?
Yes! As long as the Sliding Fee Scale chart indicates you qualify, you can take advantage of the lower cost. It doesn’t matter if you have Medicare or private insurance; if you are eligible, you save!
Will the cost savings affect my insurance deductible or stop loss?
No! The Sliding Fee Scale is a separate service that is not related to your insurance or how we bill your insurance company. It will not affect your deductible or stop loss.
Let’s say your total fee for a doctor’s appointment is $80. Your deductible is not met, so you’ll have to pay the full amount. You decide to take advantage of the Sliding Fee Scale and find out you qualify to pay just$25/visit. Even though you only pay $25, $80 will still get applied to your insurance deductible.
