Welcome to Centerstone Health Services

At Centerstone Health Services (CHS), we provide high-quality, affordable, comprehensive primary care and mental health services to help restore wellness and hope. We view the care we provide as a partnership with you to help you achieve your health goals. To assist us in providing great service to you and your family, please review the below expectations.

Prepare For Your Appointment

Bring your Photo ID, Insurance Card(s) and a list of your current medications to every appointment.

Please arrive 15 minutes prior to your scheduled appointment time to complete the check-in process or save time by completing online registration via Phreesia.

Co-Pays are expected to be paid at the time of service. Our patients are responsible for any co-insurance, deductibles, or non-covered services. For those without insurance, CHS offers a sliding fee schedule. Please ask upon arrival for assistance if needed.

Rescheduling or Cancelling Your Appointment

Your appointment time is set aside specifically for you. We ask that you give a 24-hour advance notice if you need to reschedule or cancel your appointment.

No Show Policy

After the 2nd consecutive missed appointment without 24-hour notice, you will be placed on a walk-in/same-day only status for six months and an appointment will no longer be scheduled in advance for you. Your provider(s) will be notified at this time as well. You will need to call the clinic the day you would like your appointment to see if any appointments are available.

Telehealth Appointments

Behavioral Health appointments may be scheduled with an in-person provider or via telehealth with off-site providers. Due to a shortage of psychiatry providers, we offer telehealth to provide more access to care. We require patients to be on-site for each appointment to complete health screenings, such as vitals, when scheduled with an off-site provider.

Interpreter Services

You have the right to a medical interpreter at no cost to you. Please notify our staff upon arrival for assistance.

Non-Discriminatory Care

Services will not be denied based on a person’s race, color, sex, national origin, disability, religion, sexual orientation or inability to pay.

Medications and Refills

Medication refills will only be available during regular office hours. Please allow 2-3 business days for all prescription refill requests. To avoid any disruptions to your medication regimen, we kindly ask that you do not wait until you are out of medication to make your request. All medications require an appointment and/or labs with your provider or clinical staff at specific intervals, depending on the medications. If these requirements are not met, your refill request may be denied, and we will contact you on next steps.

After Hours

For urgent needs after hours please call the clinic where you are usually seen, and the on-call nurse will assist you. NO medications will be refilled after hours.

If you have an emergency, please proceed to your nearest emergency room or call 911.
