Call the Bloomington clinic to set up your first appointment. Before your first visit, we will have you complete a new patient packet. Yes, our Sliding Fee Scale is based upon your household income and the number of people who live in your home. If our sliding fee rate is less than what your insurance leaves to your responsibility, you pay the lower rate.

Primary care providers (PCPs) are like coaches—directing and coordinating all aspects of your health care (physical, emotional, mental). They listen to your questions, assess symptoms, recommend treatments, prescribe medications, and refer you to specialists when needed.

Nurse practitioners (NPs) are advanced practice registered nurses (APRNs) with extra clinical training. They diagnose and treat medical problems, perform advanced procedures, and prescribe medications.

It’s important to have a primary care provider. They get to know you, your history, and your family health history. A long-term relationship with your PCP can mean fewer hospital/ER visits and better chronic disease management.

Be open and honest about what’s bothering you. Consider asking:
  • Are you experiencing symptoms that interfere with daily life?
  • Do you need help making healthy lifestyle changes?
  • Are sleep, stress, or family issues affecting you?
  • Is a medication making you feel unwell?
