Before your first visit, we will have you complete a new patient packet. If you have seen another family practice physician in the last 5 years we will ask you to fill out a form so that we can get your records.

I have Medicare or private insurance can I still take advantage of the sliding fee scale?
Yes, our Sliding Fee Scale is based upon your household income and the number of people who live in your home. If our sliding fee rate is less than what your insurance leaves to your responsibility then yes, you pay the lower rate.
What is a primary care provider?
Primary care providers (PCPs) are similar to coaches, directing and coordinating all aspects of your health care – physical, emotional and mental. They listen to your questions, assess symptoms, recommend treatment approaches, prescribe medication and refer you to the proper specialist when needed.
What is the role of a nurse practitioner in a primary care clinic?
Nurse practitioners (NPs) are advanced practice registered nurses who have advanced clinical training. They diagnose and treat medical problems, perform advanced procedures and prescribe medications.
Do I need a primary care provider?
It’s important to have a primary care provider. They get to know you, your history and your family health history. Building a long-term relationship with your PCP can translate to major health benefits over your lifetime such as a decrease in hospital and ER visits, and better management of chronic diseases.
What should I discuss with my primary care doctor?
Be open and honest with your provider regarding what’s going on with your health. Don’t hold back if something is bothering you.
