For urgent medical assistance after regular office hours, call the main clinic phone number and you’ll be transferred to our on-call line. No medications will be refilled after hours. For medical emergencies, dial 911 or go to the nearest emergency department. If you’re experiencing a mental health crisis, please call 988 for the Suicide and Crisis Lifeline.

You can securely message your provider through the patient portal for non-urgent questions, or call the main clinic phone number for assistance.

We offer transportation support, including gas cards, vouchers, and Uber Health. Please speak to a staff member to arrange transportation.

Nurse practitioners (NPs) are advanced practice registered nurses (APRNs) with extra clinical training. They diagnose and treat medical problems, perform advanced procedures, and prescribe medications.

We offer help around the clock – 24/7, 365 days a year. Our Crisis Response Team is staffed by highly trained, compassionate professionals and all calls are completely confidential. Community Resources

Shalom Community Center is an all-inclusive resource center in Monroe County for people living in poverty, experiencing hunger, homelessness, or lacking basic necessities. Programs: Contact Shalom at 812-334-5728 for more info.
