For urgent medical assistance after regular office hours, you may reach our on-call line by calling the main clinic phone numbers to be transferred to our after-hours line. No medications will be refilled after hours. For medical emergencies, please call 911 or proceed to the nearest emergency department. If you’re experiencing a mental health crisis, please call 988 for the Suicide and Crisis Lifeline.

You can securely message your provider on the patient portal for non-urgent medical questions or call the main clinic phone number for assistance.

Centerstone Health Services offers transportation support to help you get the care you need. We provide patients with gas cards, transportation vouchers, and utilize Uber Health. Please speak to a staff member for assistance.

You can request a medication refill on our online patient portal, or by calling the clinic. Medications will not be refilled after clinic hours.
